Partnered with X-Ray technologists and sonographers from MobileUSA, Core Care is now providing high quality digital radiography, ultrasounds, electrocardiograms and other mobile clinical services.

Members receive one of the three services listed below at NO COST each membership year. After utilizing your FREE annual imaging benefit, each subsequent test is available at the discounted rate of $199 per service! Reports will be sent directly to your prescribing physician and there is no limit on the number of tests you can receive at the discounted $199/service rate each year.

Services

As a Core Care member you will receive:

  • X-Ray – Exams are interpreted by board-certified radiologists for preliminary reports and conveniently delivered over the phone.
  • Ultrasound – Echocardiography, general ultrasound, arterial venous, and vascular doppler services are available on a scheduled basis.
  • Electrocardiogram (EKG) – Standard 12-lead electrocardiograms are performed upon request. A tracing is provided by an attending physician.

This discount card program contains a 30 day cancellation period. The plan provides discounts of 15% - 50% off only at participating providers and payment must be made at the time of purchase. You will receive a full refund of membership fees, excluding registration fee, if membership is canceled within the first 30 days after the effective date. You will receive your new identification card in the mail within 7 to 10 days after purchasing. Recurring charges are billed monthly on your membership effective date.
